Student Registration

Once you submit the online application form, please make an appointment with the Welcome Center.  You will need to bring the information below with you to that appointment along with anything else the Registration Secretary requires.

Required Residency Documents:

• A photocopy of your Child’s Birth Certificate
• Earlier School Records or Release of Records request form
• State issued identification document, Driver’s License, passport, parent’s affidavit ( parent statement as to student’s age)
• Physician’s certificate verifying student age
• Required Residency Documents list below:
  • One (1) item from Category A
  • One (1) item from Category B
  • One (1) item from Category C

**Residency Documents Submitted are required to be less than 30 days old**

Category A
MUST PROVIDE ONE

•  A current apartment or home lease agreement 
•  A mortgage document
•  Property tax record
•  Rent receipt
•  Homeowners insurance

Category B
MUST PROVIDE ONE

• A current utility bill
  • Gas Bill
  • Electric Bill
  • Water Bill 
•  Cable bill
• Home phone bill
• Insurance correspondence

Category C
MUST PROVIDE ONE

• Current proof of government benefits
  • Disability
  • Medicare
  • Supplemental Assistance Nutrition Program/ SNAP 
• Other government correspondence showing an address
• A current Connecticut driver's license
• Current automobile registration
• Current automotive insurance

*If unable to provide one item from Category B, then you must provide two (2) items from Category C*

Residency Affidavit Form

 When using this verification you must provide two (2) documents, one (1) from each of the below sections. Documents must bear the name of the person owner/lessee of the property.

**Documents Submitted are required to be less than 30 days old**

Please note, a notary does not verify proof of residency documents. 

Section 1:

• Copy of a current mortgage statement
• Copy of a current lease showing name of the lessee and tenants with landlord name and phone number

Section 2:

• Current Utility Bill
  • Gas
  • Electric
  • Landline Phone
  • Cable 

**ANY OTHER EVIDENCE, DOCUMENTS OR FORMS REQUESTED BY THE CENTRAL REGISTRATION SECRETARY NOT LISTED ABOVE**

 

WHAT YOU WILL NEED FOR YOUR REGISTRATION APPOINTMENT:

New Students:

• A photocopy of your Child’s Birth Certificate
• State issued identification document, driver’s License, passport, parent’s affidavit ( parent statement as to student’s age)
• Custodial Agreement (only if parents/ guardians do not live together but share custody or if only one parent has sole custody) from the court
• Currently Physical
• Current Immunizations
• Two(2) Residency Documents less than 30 days old
• If your child/children have attended another school
  • Signed/Completed Permission to Release/Obtain Records Form (Spanish)
